Características clínicas y manejo inicial del síndrome coronario agudo en el servicio de emergencia de un centro privado de Lima, Perú: estudio retrospectivo 2018–2023

Abstract

Introducción: El síndrome coronario agudo representa una de las principales emergencias cardiovasculares y requiere una evaluación clínica, electrocardiográfica y bioquímica oportuna para orientar el manejo inicial. En el Perú, la información sobre sus características clínicas y decisiones terapéuticas en instituciones privadas es limitada. Objetivo: Describir las características clínicas, epidemiológicas y el manejo inicial de pacientes con síndrome coronario agudo atendidos en el servicio de emergencia de un centro privado de Lima, Perú. Métodos: Se realizó un estudio observacional, descriptivo y retrospectivo en pacientes adultos con diagnóstico confirmado de síndrome coronario agudo atendidos entre 2018 y 2023. Se recolectaron variables demográficas, comorbilidades, presentación clínica, subtipo de síndrome coronario agudo, biomarcadores cardíacos y decisiones terapéuticas iniciales. Resultados: Se incluyeron 35 pacientes. La mediana de edad fue de 62 años y predominó el sexo masculino. La hipertensión arterial fue la comorbilidad más frecuente, seguida de diabetes mellitus y cardiopatía isquémica. El subtipo clínico más común fue el infarto agudo de miocardio sin elevación del ST, seguido del infarto agudo de miocardio con elevación del ST y la angina inestable. El número de casos registrados fue menor durante 2020 y 2021. En cuanto al manejo inicial, el 40,0% de los pacientes recibió trombólisis y el 62,9% fue sometido a cateterismo cardíaco. Conclusión: En este centro privado, los pacientes con síndrome coronario agudo fueron predominantemente varones, con alta frecuencia de factores de riesgo cardiovascular y uso frecuente de estrategias de reperfusión o manejo invasivo. Estos hallazgos resaltan la importancia de fortalecer el registro clínico, la estratificación inicial y los circuitos de atención oportuna en emergencia.
